Youth Retreat 2022: Leaders

Cost
$50 per leader: Application and payment due June 5

Payment can be made by Zelle or check, with Youth Retreat 2022 on the memo line for both.

Use offering@jubileechurch.us to find us on Zelle. Checks should be made out to Jubilee Church and placed in the Sunday offering bin or mailed to the Kwans at 1300 Annalea Drive • McKinney, TX 75071.

​Please note that retreat payment cannot be reimbursed if you cancel your leader application after June 19.

    Leader Application

    RELEASE AND POLICY AGREEMENT
    By volunteering as a leader for Jubilee Church’s 2022 Youth Retreat, I agree to be bound by the following agreement.

    Photo, Audio, and Visual Presentations Release. I understand that photographs, audio and/or video recordings may be taken of me while attending Jubilee Church’s 2022 Youth Retreat. I hereby give my consent and permission to Jubilee Church to use my likeness and voice in any manner, including but not limited to, publishing, reproducing, exhibiting, and broadcasting through any print and electronic media, including the internet, social media, and Jubilee Church’s website. I hereby forever release Jubilee Church, its successors, officers, employees, agents, and assigns from any claims, damages, or demands whatsoever that may arise by reason of such use.

    General Release. I hereby release and forever discharge and hold harmless Jubilee Church and its successors, officers, employees, agents, and assigns from any and all liability, claims, and demands of whatever kind or nature, including but not limited to, bodily and personal injuries or illness or damage to property that arise from or occur by reason of my participation in Jubilee Church’s 2022 Youth Retreat.

    Group Dynamix Release. I understand that, if I choose to participate in the Group Dynamix outing, I will need to sign a consent form and waiver required by Group Dynamix in order for me to participate in its provided activities.

    Background Check Assent. I assent to the administration of a background check of my criminal record and, if applicable, my driving history before my application to volunteer at Jubilee Church's 2022 Youth Retreat is approved. I agree to provide any personal information necessary in order to perform the background check. I understand that the results of the background check will be used to determine whether or not I am approved as a retreat leader, and my retreat payment of $50 will be reimbursed if I am not approved.

    I agree that typing my name below is considered equal to providing my signature. By signing below, I acknowledge that I have read and understand the Release and Policy Agreement in full and agree to abide by it.
