RELEASE AND POLICY AGREEMENT
By attending Jubilee Church’s Youth Retreat, we agree to be bound by the following agreement.

Photo, Audio, and Visual Presentations Release. We understand that photographs, audio and/or video recordings may be taken of my children during their attendance at Jubilee Church’s Youth Retreat. We hereby give our consent and permission to Jubilee Church to use our likenesses and voices in any manner, including but not limited to, publishing, reproducing, exhibiting, and broadcasting through any print and electronic media, including the internet, social media, and Jubilee Church’s website. We hereby release Jubilee Church, its successors, officers, employees, agents, and assigns from any claims, damages, or demands whatsoever that may arise by reason of such use.

General Release. We hereby release and forever discharge and hold harmless Jubilee Church and its successors, officers, employees, agents, and assigns from any and all liability, claims, and demands of whatever kind or nature, including but not limited to, bodily and personal injuries or damage to property that arise from or occur by reason of our participation in Jubilee Church’s Youth Retreat.

We agree that typing our names below is considered equal to providing our signatures. By signing below, we acknowledge that we have read and understand the Release and Policy Agreement in full and agree to abide by it.
